What is GUIDE?

GUIDE (Guiding an Improved Dementia Experience) program from the Centers for Medicare & Medicaid Services is an innovative initiative aimed at transforming the care landscape for individuals with dementia and their caregivers. This model tests a new approach to dementia care that integrates comprehensive care coordination with a focus on enhancing the quality of life for patients and reducing the burden on caregivers.

Comprehensive Care
GUIDE offers personalized care planning and ongoing health monitoring to meet each patient's unique needs, fostering coordinated healthcare efforts for comprehensive health management.
Respite Care
Services can be delivered in-home or at a facility. This recognizes the demands placed on caregivers and allows them to take breaks to help maintain their health and well-being.
Cost Coverage
All services under the GUIDE program are available at no additional cost to Medicare beneficiaries, ensuring that finances do not hinder access to necessary care.

For Patients and Caregivers

GUIDE is a Medicare benefit that covers a variety of clinical services focused on dementia care. The program supports patients and caregivers by providing services such as comprehensive assessments, care planning, ongoing monitoring and support, medication management, 24/7 access to care teams, and caregiver education & support.

If you are a traditional Medicare beneficiary and you would like to learn if you are eligible for GUIDE services, Harmonic Health clinicians can perform a comprehensive assessment and evaluation. If they determine you meet the diagnosis criteria for GUIDE they will facilitate your enrollment and ongoing participation in GUIDE. Your primary care provider or neurologist can refer you to us, however, you do not need a referral to access services.Contact us at support@harmonichealth.com for guidance on how to enroll.

GUIDE services are covered under Medicare at no additional costs for the eligible services specified under the model.

GUIDE enhances access by providing a structured care model that includes home visits, caregiver education & support, clinical care recommendations, and management provided by your dedicated HH clinical team. They will work with your existing care providers to coordinate your care.

For Providers

GUIDE (Guiding an Improved Dementia Experience) is a model from the Centers for Medicare & Medicaid Services that aims to

  • Improve the quality of life for people living with dementia by addressing their medical, behavioral, and functional needs, coordinating their care for dementia and co-occurring conditions, and improving transitions between community, hospital, and post-acute settings.
  • Reduce the burden and strain on unpaid caregivers by providing skills training, referrals to community-based social services, 24/7access to a support line, and respite services.
  • Prevent or delay long-term nursing home care for as long as appropriate by supporting caregivers and enabling people living with dementia to remain safely in their homes for as long as possible.

By collaborating with Harmonic under GUIDE, providers can enhance care coordination, receive support through caregiver education programs, and access respite services payments for eligible beneficiaries. This collaboration can reduce hospital & emergency visits, and potentially lower long-term care costs.

GUIDE offers a per beneficiary per month care management payment (DCMP), adjusted for health equity and performance on quality metrics. Additionally, separate payments are available for respite services for beneficiaries with moderate to severe dementia.
